How to refer a patient

01

02

We verify & coordinate

Our team verifies insurance benefits, obtains any required prior authorizations, reviews documentation for completeness, and contacts the patient to schedule their fitting.

If the patient is outside our service area and unable to come to our office, we can process the order using current measurements provided by their physician or certified lymphedema therapist and ship the product directly to the patient.

03

Expert fitting

A certified fitter performs a personalized assessment, takes measurements, and recommends the most appropriate product based on the provider's prescription, the patient's clinical needs, and insurance coverage.

04

Fit. Educate. Follow up.

After the product arrives, we complete the fitting, educate the patient on proper wear, care, and use, and ensure they are comfortable with their new garment. We continue to support our patients after the fitting and help manage eligible reorders and replacements as their treatment continues.

Information about insurance

We're contracted with most major insurance plans. Because coverage varies by policy, we verify each patient's benefits, obtain any required prior authorizations, and review any coverage limitations or expected patient responsibility before an order is placed.

Expert compression and mastectomy care for your patients

"I've been recommending patients to P&H Services since I was a physical therapy student, and I still do today. Even when my patients and I are in a different city, I refer them without hesitation — I know they'll receive top-notch, personalized care without any hassle. Their staff takes the time to educate patients, address concerns, and get the product and fit right. I recommend them to both patients and providers looking for reliable, compassionate, professional care."

Shaniece D. Witte, PT, DPT, CLT-LANA, Cert. DN
Certified Lymphedema Therapist

"As a Certified Lymphedema Therapist, I have relied on P&H Services for 10+ years to deliver reliable care to my patients. Their expertise and attention to detail ensure my patients receive top level care. They are professional, responsive, and consistently exceed expectations. I highly recommend their services to anyone looking for a team they can trust."

Jessica M. PT, DPT, CLT, CORE
Certified Lymphedema Therapist

"For the last 9 years, P&H Services has consistently shown up to help service my patients in a more rural setting almost 100 miles away from their locations. They have excellent, personalized customer service for myself as a clinician as well as to my individual patients. The leadership of P&H Services have diligently worked to understand and integrate the LTA legislation, allowing wonderful access to products for our patients on Medicare plans! I have enjoyed working with them — they provide the highest quality vendors and don't cut corners or take risks for patient financial responsibility. They coordinate with my clinical recommendations and balance that with patient preferences. Will continue to recommend my patients connect with them for compression garment needs!"

Have questions?

How do I refer a patient?

To refer a patient, please send us the patient's demographics, a valid prescription, and supporting clinical documentation from a recent office visit that supports the item being prescribed.

Referrals can be sent by:
Fax: (512) 986-6330
Email: reception@phservnet.com

Once we receive the referral, we'll verify benefits, obtain any required authorizations, and coordinate the fitting with your patient.

For product-specific documentation requirements, please see our Compression and Mastectomy Documentation Requirements Guides.

Are appointments required?

Yes. Scheduled appointments allow us to verify insurance benefits, obtain any required prescriptions and supporting documentation before the visit, ensure a certified fitter is available, and dedicate the time needed for a comprehensive evaluation and fitting.

If a patient arrives without an appointment, we'll do our best to accommodate them. However, because insurance verification, documentation, and fitter availability can vary, same-day service cannot be guaranteed.

Can I refer a patient who lives outside your service area?

Absolutely. We regularly work with patients outside our local area.

To get started, please send the patient's demographics, a valid prescription, supporting clinical documentation from a recent office visit, and current compression garment measurements. Once we receive the referral, we'll verify benefits, obtain any required authorizations, and process the order.

Please send referrals to:
Fax: (512) 986-6330
Email: reception@phservnet.com

Which insurance plans do you accept?

We're contracted with Medicare, Texas Medicaid, and many commercial insurance plans. Before placing any order, we verify your patient's specific benefits, review any expected out-of-pocket costs with the patient, and obtain any required prior authorizations so there are no surprises.

For a current list of the insurance plans we accept, please see our Insurance Plans page.
